Sickness is a visual novel that follows the life of Suoh, a high school student turned violent criminal. After the death of his parents, Suoh is left without a home and without a source of income. All he has is his twin sister Sara, whom he promises to take care of, even if it means throwing his life away. With all their savings, the two move to another city, where Suoh quickly finds work. Sara commutes to her old school, Suoh works long, physically demanding hours, and the two survive on convenience foods and barely make ends meet. It’s not a happy life, but they make it work. Or at least they did until a violent eruption set in motion a rapid descent. In the midst of a physical altercation with his boss, Suoh passes out upon waking to find a dead body lying at his feet. His ankles bleed, his muscles ached as he realized he had just murdered his boss, who is responsible for his continued well-being. But instead of lamenting the loss of life, all Suoh can think about is how badly he needed the money and what he will do next to get it. Attracted by the smell of blood, one of Suoh’s colleagues, a young man with criminal connections, offers him a not-so-legal revenue stream that encourages violent behavior and rewards the able-bodied. And from there everything gets out of hand. It turns out Suoh has a knack for crime, and the little reservations he had about such acts don’t last long. For his sister’s well-being and his own interest, he becomes a hitman and kills everyone his new boss tells him to do. He quickly adapts to life as a murderer, adopting a double life that his sister can’t know about. Will Suoh survive long enough to keep his promise to his sister? Will he give up and escape this life of crime and find solace in the company of someone or something else? Or will he be hunted down and killed before he even gets a chance?
